In summary, although the rate of death did not differ significantly between the group of patients treated with dopamine and the group treated with norepinephrine, this study raises serious concerns about the safety of dopamine therapy, since dopamine, as compared with norepinephrine, was associated with more arrhythmias and with an increased rate of death in the subgroup of patients with cardiogenic shock.

The type of shock that was seen most frequently was septic shock (in 1044 patients [62.2%]), followed by cardiogenic shock (in 280 patients [16.7%]) and hypovolemic shock (in 263 patients [15.7%]). The sources of sepsis are detailed in Table 2 in the Supplementary Appendix. Hydrocortisone was administered in 344 patients who received dopamine (40.1%) and in 326 patients who received norepinephrine (39.7%). Among patients with septic shock, recombinant activated human protein C was administered in 102 patients in the dopamine group (18.8%) and 96 patients in the norepinephrine group (19.1%).
Since the magnitude of the effect derived from observational studies can be misleading, we opted for a sequential trial design with two-sided alternatives20; the trial design called for analyses to be performed after inclusion of the first 50 and 100 patients, and then after inclusion of each additional 100 patients, and allowed for the discontinuation of the trial according to the following predefined boundaries: superiority of norepinephrine over dopamine, superiority of dopamine over norepinephrine, or no difference between the two. An independent statistician who is also a physician monitored the efficacy analyses and the adverse events; on October 6, 2007, after analysis of the outcome in the first 1600 patients showed that one of the three predefined boundaries had been crossed, the statistician advised that the trial be stopped.

Athletes often do depth jumps with two plyo boxes: one to step off of and another to jump onto. Essentially, it’s a depth jump into a box jump. When doing this variation, make sure to leave enough room between the boxes to allow you to land and jump safely (3–5 feet between boxes should work). Though improving jumping technique may add a couple inches to an athlete's vertical jump, good landing technique is even more crucial. The landing is when almost every jumping-related injury occurs, not the jump itself. For this reason, athletes should spend a significant amount of time learning to land in a balanced position that distributes the impact of the jump equally across all joints of the lower body. This position should look almost identical to the take-off position.

“There’s something about dunking a basketball that lures us in,” he said, reflecting on his first jam, during lunch period his sophomore year at De La Salle High in Concord, Calif., back when his driver’s license read 5' 11", 112 pounds. “It stokes the imagination. The baseline dunk is an approach-modifier of any dunk type in which the player approaches the basket along the court-boundary (baseline) which runs parallel with the backboard. Stand on the bottom step of a flight of stairs holding the railing on either side. Place the balls of your feet on the stair with your heels hanging off the edge. Slowly raise your heels as high as you can, and hold for two seconds. Slowly lower your heels below your toe level, and hold for two seconds. Repeat calf raises exercise 20 times for three sets with a 1-minute break between each set.

Unfortunately, I’m not the 6' 7" son of a Hall of Famer, so I had to resort to desperate devices—like Hennessy, an infamous and inexpensive cognac that, according to one of the two NBA players who recommended it to me, “will give you that Yah! That bounce. That little bit of meanness you need.” The little minibar-sized bottle that I downed 30 minutes into an intense session of dunk attempts on a sweltering day last summer, had no effect other than scorching my esophagus, giving me a headache and releasing from my pores an aura that, as my six-year-old put it that evening, “smells like medicine.”

My quest to dunk started poorly. The main problem was that I could only do about half of the very long list of ercises the Jump Manual instructed at the crowded and inadequate YMCA near my place. The basketball court—the only space big enough to do some of the drills—was always occupied with classes. The Strength Shoes, meanwhile, were so absurd that I was too embarrassed to wear them in front of other gym-goers. I used them only a handful of times, in an empty stairwell on the top floor of the gym.


At pickup the next night, buoyed by the previous day’s accomplishment, I found a regulation ball that had good grip, one I could palm, and in between games, when no one was looking, I dunked for the first time in eleven years. If some dunks are described as thunderous, this one could be best described as a gentle fart in the breeze. But a dunk’s a dunk—and I had dunked.

Generally, a player can reach their highest when jumping off one foot and reaching up with one hand. For a player that is right-handed, the most common way is approaching from the left and jumping off the left foot with the ball in the right hand. Obviously, for a left-handed player, it’s coming from the right and jumping off the right with the ball in your left hand.


I found out that if you practice penultimate two-step jumping practice dunking or just jumping, it gives a MUCH better workout than any one step jumping practice. I was also able to increase my one foot jump vertical by this. I think 2-step dunking is much more powerful and waaaaaay cooler. It is also much safer because you are in more control of your body. Right now I'm 6'1" and can 2-step tomahawk and basically everything off one foot

About 100 yards away from this 9' 10" breakaway rim (which came to sound, each time I grabbed and released it, like someone closing the metal baby gate at the top of our stairs) was a brown, oxidized, immobile 9' 1" version, a hand-ruining iron maiden where, in front of the occasional puzzled onlooker, I practiced (and practiced) the timing and the hand and wrist work required to dunk. I knew early on that my regulation dunk, if it ever came to pass, would have to come from a lob of some sort—a bounce to myself, either off the blacktop or underhanded off the backboard—after which I would hypothetically control the ball with one hand just long enough to flush it. Secondly, in addition to the rate of force development, the size of the force itself produces a negative feedback effect on vertical impulse, because higher forces lead to faster accelerations, which in turn reduce the time spent producing force before take-off. This is *partly* why drop jumps tend to involve higher forces, shorter ground contact times, and yet similar jump heights to countermovement jumps.
